Attach the Quality Course Design rubric to which you have added comments that summarize how your course meets each of the criteria. Download the rubric.
Ensure that you have followed all of the steps outlined in the Submission Directions section of the Award for Excellence in Online Teaching page. Nomination forms that do not include the required documentation (Quality Of Course Design Rubric and Student Feedback on Course Effectiveness Report) will not be considered.
